The Horizon Foundation’s Speak(easy) Howard campaign will host a FREE webinar series on incorporating advance care planning into health care settings. The sessions are designed for health care professionals. This webinar series normally costs more than $700, but is being offered at no cost to participants.
Through six weekly sessions led by national experts with the Institute for Healthcare Improvement and The Conversation Project, participants will learn about:
- Reliably stewarding advance care planning information across the health care system, including strategies for working with electronic health records.
- Engaging communities that help to activate the public in having these conversations in advance of a potential medical crisis.
- Making use of CMS reimbursement for advance care planning conversations.
Health care providers (primary care, hospital-based, palliative care, elder care, nursing home, long-term care, skilled nursing facility or hospice care), health system leaders, social workers, chaplains and other clinical staff are all invited to participate and learn tools to address advance care planning in their role. While the time period to register for CEUs has passed, all are still welcome to join and listen in.
